Submit three documents, everything will be done as per your wish.
When you are filling online application for correction in your name, EPFO will ask for online documents from you. You will have to provide only three documents. That too is not a special document. EPFO will give you a list of 19 documents. You have to attach any three of those documents. Yes. Make sure to check that all three documents support the type of correction you want in your name. These 19 documents are – Aadhaar, Passport, Death Certificate, Birth Certificate, Driving License, Service Photo Identity Card of Central or State Government or any of their Bank or PSU, Bank Passbook, PAN Card, Ration Card, Voter ID, Pensioner Photo ID, Any kind of medical claim card, caste certificate, gazette notification of change of full name or first word of name, along with foreign passport for foreigners. Visa, Freedom Fighter Card, Person of Indian Origin Card, Overseas Citizen of India Card, Tibetan Refugee Card. In these only Aadhar card is mandatory. You can take any other identity from the remaining two in the list.
